Case Management

 

 

 

Overview: The primary role of a case manager is to coordinate the continuity of care and to ensure that patients get the proper treatment at the proper time to maximize health and minimize hospitalization. The nursing case management framework includes assessment, planning, implementation, evaluation, and interaction. Case managers can work with patients of all ages and diagnoses, but they usually specialize in a particular area, such as HIV/AIDS, rehabilitation or geriatrics.

Typical venues: long-term care facilities, health care organizations, HMOs, community health organizations, behavioral health programs, rehab centers, schools, case management companies

Job outlook: The Bureau of Labor statistics predicts that nursing as a whole will be one of the top 10 fastest growing professions in the United States in the next decade. The need for case managers is likely to grow rapidly as the ever-increasing number of elderly patients require help with managing their chronic conditions.
