Assessment: Phase 1

Accomplishment Statement

This is a statement summarizing achievements, projects or results you've produced and are proud of. For example, did you establish a procedure or protocol that made a difference in patient care? Did you serve on a special task force aimed at improving quality or nursing standards? Did you save your unit or floor money? Did you receive an award or recognition? Then you write a statement summarizing the results.

To do this, first identify the project or task, second write up the action steps you took to resolve the need or problem, and third write up the results produced. Then write an action statement summarizing the results of your efforts. See the following example for the PAR method, Problem Action Result.

1. Problem: Patient call lights and complaints are not being responded to in a timely manner. Customer satisfaction survey scores are down 20 percent.

2. Action: Met with patient care team and devised strategies for answering call lights and communicating more effectively with patients on the unit.

3. Result: Patient complaints went down, and customer satisfaction scores increased by 30 percent.

4.Summary statement: "Successfully worked with patient care team to resolve patient complaints in a timely manner, resulting in customer satisfaction survey score increasing by 30 percent."
